The Survey Activity for Forj Learn Courses can be used to collect valuable feedback from Learners on their impressions of a course. There are several different question types available from the Survey Activity, including: rate scale, short answer, long answer, and multiple choice.
Data from the Survey Activity can be viewed by Learning Admins per Question or per Learner. Learning Admins can review the results and follow-up with Learners.
Note it is not recommended to make changes to the Survey Activity in a Published course. This is to maintain integrity of questions to answers provided by Learners.

SURVEY ACTIVITY SETUP
Follow the steps below to setup the Survey Activity:
Once within the course, navigate to the Outline page.
- From the Activity Type dropdown, select Survey.
- Name the Survey Activity by entering text in the Activity Title text box, then click the Add button.
- Enter information about the Survey into the Description text box. Admins can alert Learners to the number of questions, the question topics, or the amount of time expected to complete the Survey.
- There is no character limit, and special characters can be used.
- There are formatting options to add header sizing or bold, underline, or italicize your text. Numbered or bullet point lists can be created, and links, emojis, images, videos, and documents can be added. Highlight any text to add a link.
- From the Add a question section select a question Type from the dropdown list.
SHORT ANSWER
The Short Answer question type allows Learners to respond with a short sentence or phrase.
- Enter the question text into the Question text box.
- There is no character limit, and special characters can be used.
- There are formatting options to add header sizing or bold, underline, or italicize your text. Numbered or bullet point lists can be created. Highlight any text to add a link.
- Determine if the question is required and toggle on or off as needed.
- Add another question as needed or click the Save button.
LONG ANSWER
The Long Answer question type allows Learners to respond with several sentences.
- Enter the question text into the Question text box.
- There is no character limit, and special characters can be used.
- There are formatting options to add header sizing or bold, underline, or italicize your text. Numbered or bullet point lists can be created. Highlight any text to add a link.
- Determine if the question is required and toggle on or off as needed.
- Add another question as needed or click the Save button.
MULTIPLE CHOICE
The Multiple Choice question type allows Learners to select one or more answers from a list of choices.
- Enter the question text into the Question text box.
- There is no character limit, and special characters can be used.
- There are formatting options to add header sizing or bold, underline, or italicize your text. Numbered or bullet point lists can be created. Highlight any text to add a link.
- Click the Add option button to enter an answer choice.
- Enter the answer text into the text box.
- There are formatting options to add header sizing or bold, underline, or italicize your text. Numbered or bullet point lists can be created. Highlight any text to add a link.
- Click the trash can icon to delete an answer if desired.
- Determine if the question is required and toggle on or off as needed.
- Click the Save button.
RATING SCALE
The Rating Scale question type allows Learners to select their preference between a predefined range of options.
- Enter the question text into the Question text box.
- There is no character limit, and special characters can be used.
- There are formatting options to add header sizing or bold, underline, or italicize your text. Numbered or bullet point lists can be created. Highlight any text to add a link.
- Enter the number of radio buttons available in the scale.
- The default is a Scale from 1 to 5.
- Enter the text to describe the scale for rating.
- Examples are from Unacceptable to Outstanding.
- Determine if the question is required and toggle on or off as needed.
- Click the Save button.
LEARNER VIEW
Learners will view the Survey questions directly on the course page. Required questions will need to be answered for the Learner to Submit their answers.
Once Learners have completed the course, Learning Admins can view the results of their Survey responses. Click here for documentation on how to access the Survey Activity results.
